How do you setup DSL for Qwest?

I purchased a 2wire 1701HG DSL modem for a friend in another state.  This was purchases on Ebay, and there was no disc with it. We know his account name and password. But we don't know whether to use PPoE or PPoA, and there are five key codes suggested for it, but we don't know which one to use. Does the setup require windows network setups? He is using an Ethernet cable to connect to the 2Wire gateway modem.
